In-Office Laboratory Tests

f payable by Medicaid, the laboratory tests listed and attached below are allowed to be performed in a physician’s office. All other lab tests must be referred to our in-network lab, Quest Diagnostics.

Note: If a laboratory test performed in the physician’s office is not listed above, the claim will be denied with the following message: “Services covered only when performed by in-network laboratory.”
